Surgical options to treat gum diseaseGum disease need not be a reason for losing your teeth. Non-surgical treatments like scaling and root planing treat mild forms of gum disease. But advanced gum diseases need treatments beyond just scaling and root planing. In such cases, these basic treatments are followed by surgery. While non-surgical treatments control the bacterial growth and progress of the gum disease, surgical treatments restore your teeth’s supportive tissues. Curettage
Curettage involves removal of inflamed and dead gum tissues with special instruments to promote the development of healthy tissue. Under local anaesthesia, your dentist will reflect away the gum tissue from the teeth and remove the inflamed tissue surrounding them. While doing so, he/she will also remove the tartar deposits present on the root surface and correct the irregularities in the bone. The gum is then repositioned back against the teeth. Gingivectomy (Gum excision)
Excision (or removal) of the diseased pocket wall of the gums is known as gingivectomy. It provides visibility and helps access the root surface of the teeth in order to remove deposits and smoothen the roots. Removal of the diseased gum enhances gum healing and helps reshape it. Flap surgery
Flap surgery is done to reduce the deep periodontal pockets. It involves lifting back a section of the gums. This enables visibility of and access to the root of the tooth and the bone surrounding it.After the area is thoroughly cleaned of all tartar and debris, the flap is placed at the desired position and sutured.

Grafts
Grafts help reverse some of the damage caused by gum disease. They are used to regenerate the bone or gum tissue lost to gum disease. Exposed tooth roots due to gum recession may be covered by soft tissue grafts which may be taken from your palate or some other donor source. When the bone supporting your teeth has been destroyed a bone graft may be done. During a graft procedure, your dentist will retract the gum tissue, remove the tartar, clean the area and place membranes, growth factors (enamel matrix protein derivative), gum tissue grafts and/or bone grafts to regenerate and restore gum and/or bone.

Guided tissue regeneration
It is a technique used with bone grafts. A small piece of membrane, either degradable or non-degradable, is inserted between the gum tissue and bone. The membrane inhibits the gum tissue from growing into the bone area thereby allowing the growth of bone and connective tissue.
