Diagnosing drug-resistant TB could now be faster

Getting treated for drug-resistant TB is undoubtedly one of the biggest challenges India is facing out right now. And the fact that the diagnosis of drug-resistant tuberculosis takes a few months time has lead to an increase in the number of cases of multidrug-resistant TB (MDR-TB) and extensively drug-resistant TB (XDR-TB) in the country. Although the latest tool to diagnose drug-resistant TB now takes only three months, it often becomes difficult to get treated, given the time interval between the two diagnosis and treatment. However, with an aim to lower the time required to get diagnosed for drug-resistant tuberculosis, a team of British researchers has developed a new technique, which can detect the bacteria-causing tuberculosis within few days. Is it better than the current conventional methods?

#1 Conventional method: Apart from taking few months’ time to detect the bacteria, these tests are very costly. Also, the collection of the sample and sending it to for testing is a time-consuming process. And since the diagnosis takes time, the treatment is delayed. The common diagnostic tests to detect drug-resistant TB are:

  • Xpert MTB/RIF (sputum)
  • Genotype MTBDRplus V2, (sputum and culture material)
  • MGIT Liquid Culture

#2 The current method: The new method allows scientists to enrich Mtb DNA directly from patient sputum (mucus) samples. The samples can be sequenced and analysed straight away, avoiding the need to spend weeks growing them in the lab. Apart from delivering personalised treatments to patients, the tests could also be used to precisely track the spread of TB. (Read: DOTS and its 5 basic measures to deal with tuberculosis)

Will this technique improve the survival chance?

With the help of conventional methods, patients with resistant TB would need to wait for up to six weeks for antibiotic resistance testing. However, this new technique and the associated software could reduce testing for antimicrobial resistance to a few days. It also allows the doctors to start precise antimicrobial treatment earlier than what is currently possible. This in turn, not only improves the effectiveness of the treatment but also enhances the survival chance of TB patients.

The study was published in the Journal of Clinical Microbiology.
