Blood Cancer Awareness Month: How Have Advancements In Treatment Improved Patient Outcomes?

Blood cancers or hematologic cancers affect the blood, bone marrow, and lymphatic system. They can vary significantly in their nature, presentation, and prognosis. Understanding the types of blood cancer, their signs and symptoms, potential prevention strategies, and available treatments is crucial for timely detection and effective management. More than 70,000 people in India died from blood cancer, according to the Globocan 2022 report. The report also highlighted a significant increase in diagnosed cases, rising to 1,20,000 from the previous 1,00,000. Notably, 30,000 of these cases are in children.
Types Of Blood Cancer
Blood cancers primarily fall into three main categories: leukemia, lymphoma, and multiple myeloma.
Leukemia: This is a cancer of the bone marrow and blood. Leukemia can be acute (fast-growing) or chronic (slow-growing) and is further classified into types such as Acute Myeloid Leukemia (AML), Acute Lymphoblastic Leukemia (ALL), Chronic Myeloid Leukemia (CML), and Chronic Lymphocytic Leukemia (CLL). Acute leukemias are life-threatening and require immediate treatment, whereas chronic leukemias may be monitored and treated based on the progression of the disease.
Lymphoma: This cancer begins in the lymphatic system, which is part of the body's immune system. Lymphomas are divided into two main types: Hodgkin lymphoma and non-Hodgkin lymphoma. Non-Hodgkin lymphoma is more common and has several subtypes, each with different characteristics and treatment approaches.
Multiple Myeloma: This is a cancer of the plasma cells, a type of white blood cell found in bone marrow. Multiple myeloma affects the body's ability to produce healthy blood cells and antibodies, leading to weakened bones, kidney damage, and a compromised immune system.
Causes And Risk Factors
The exact causes of blood cancers are not well understood, but several risk factors have been identified:
Genetic predisposition: While most blood cancers are not inherited, some rare cases have a genetic link.
Exposure to certain chemicals: Chemicals such as benzene and herbicides like glyphosate have been associated with an increased risk of developing blood cancers.
Radiation exposure: Previous exposure to high levels of radiation, such as through certain cancer treatments or nuclear accidents, can increase the risk of blood cancers.
Age and gender: The risk of blood cancers generally increases with age and is slightly higher in males than in females.
Viral infections: Some viruses, like the Epstein-Barr virus, have been linked to certain types of lymphoma.
Symptoms Of Blood Cancer
The symptoms of blood cancer can be vague and non-specific, often resembling those of other, less serious conditions. Common symptoms include:

These symptoms may vary depending on the type and stage of the blood cancer. Due to the non-specific nature of these symptoms, blood cancers are often diagnosed at an advanced stage when the disease has progressed.
Screening And Early Detection
Early detection of blood cancers can significantly improve the chances of successful treatment. Some cancers can be detected at an early stage and some blood cancers, such as Chronic Lymphocytic Leukemia (CLL) and Multiple Myeloma, can sometimes be detected through routine blood tests, like a complete blood count (CBC), even before symptoms develop. For individuals with risk factors or a family history of blood cancer, regular monitoring and blood tests can aid in early detection.
Prevention
Currently, there are no guaranteed methods to prevent blood cancer, but certain lifestyle changes may reduce risk:
Avoiding exposure to harmful chemicals: Limiting exposure to known carcinogens like benzene and glyphosate can reduce the risk.
Maintaining a healthy lifestyle: A balanced diet, regular exercise, and avoiding tobacco can support overall health and potentially lower cancer risk.
Avoiding unnecessary radiation exposure: Minimising exposure to radiation through medical imaging and other sources can help reduce risk.
Advances In Treatment
Significant advances in the treatment of blood cancer have been made over the past decade, leading to improved outcomes and survival rates. Treatment strategies are tailored based on the specific type of blood cancer, the patient's risk profile, and the presence of specific genetic mutations or abnormalities.
Chemotherapy and targeted therapy: Conventional chemotherapy remains a cornerstone of blood cancer treatment. Newer targeted therapies, which attack specific genetic changes in cancer cells, have greatly enhanced treatment precision and effectiveness.
Immunotherapy: This treatment uses the body's immune system to fight cancer. Newer forms of immunotherapy such as CAR T-cell therapy have shown promise in treating certain types of leukemia and lymphoma that do not respond to traditional therapies.
Stem cell transplantation: Also known as bone marrow transplantation, this intensive treatment can potentially cure high-risk leukemias and lymphomas by replacing diseased bone marrow with healthy stem cells from a donor.
Risk stratification and personalised medicine: Advanced cytogenetic and molecular testing allows for the classification of patients into different risk categories. For example, leukemia treatment varies by risk level as low-risk cases may receive less intensive therapy, while high-risk cases require aggressive treatments like targeted therapies and bone marrow transplantation. This stratification helps in tailoring treatments according to the individual's risk profile.
CAR T-cell therapy: A groundbreaking treatment that involves modifying a patient's T-cells to attack cancer cells, this therapy has shown significant success in treating certain blood cancers that are resistant to other treatments.
Blood cancers are complex and vary significantly in their presentation and behaviour. Understanding the types, risk factors, symptoms, and available screening methods is essential for early detection and effective treatment. While prevention strategies remain limited, advancements in therapies have revolutionised treatment options, providing hope for many patients. Continued research and development are crucial to further improve the outcomes and quality of life for those affected by blood cancers.
